I deleted Users/username/Library/Application Support/gephi/0.9.1/dev/config
Started gephi, loaded my network file, clicked "Preview" and "Refresh". And there's the preview!

Deleting the config directory, restarting gephi, and loading the file worked! I now have previews.

Great! It seems sometimes preview can end up in a broken state for the user configuration.
